behalf of the above-named team. I hereby authorize the directors
and agents of the National Double Dutch League to act in my behalf
using their best judgment in any emergency, including medical
emergency. I hereby waive and release the National Double Dutch
League, its employees, volunteers, directors, sponsors, suppliers,
and facilities from any and all liability for illness or injury
incurred while attending the 15th Annual Double Dutch Holiday
Classic. I know of no mental of physical problems that might adversely
affect my ability to participate in this event. I hereby grant
NDDL permission to use any photographs or video of me/my team
for promotional purposes. I have read the Official 2006 Holiday
Classic Rules and Guidelines packet and agree to its terms and
conditions including the refund policy. I understand I must still
submit in writing, official release forms signed by the parents
or the legal guardians of the above-named paraticipants. |
